Effective Programs that Reinforce our Mission

Safety

Traditions Health Care ensures nursing homes shall be structurally safe, well equipped, and maintained to protect the health and safety of residents, personnel, and visitors. Monthly we conduct a preventative maintenance audit. This audit checks essential mechanical, electrical, and resident care equipment to verify that it is operating correctly. Any piece of equipment that is deemed faulty is fixed and or replaced as necessary for resident safety. We ensure that all regulatory standards regarding safety measures are maintained. we conduct a safety meeting monthly. The safety committee members representing each department report any concerns or recommendations. The information reported is promptly addressed and a plan implemented to correct any problems.

 

Infection Control

Traditions Health Care has a program to prevent the transmission of infection. We take this very seriously! We have ongoing staff training that continually educates the staff on appropriate infection control practices. We hold an Infection Control Committee meeting monthly to address current concerns, any trends and a review of written policy and procedure. Additional educational requirements that may be needed are addressed and a plan implemented to correct any deficient practices. We have established a relationship with an advanced practice nurse who specializes in infection control.

 

Nursing Services

The nursing homes maintain an organized nursing service with qualified nursing personnel to meet the needs of all residents 24 hours a day. Nursing care must meet the following criteria: consistent Quality of Care provided by the employees; sufficient staffing to compassionately carry out the care, safety and dignity of each resident; coordinating plans of care developed by the interdisciplinary team and monitored by the physician; nutritional needs of each resident are met and monitored.

 

Rehabilitation Services

Traditions Health care establishes an active program of rehabilitation nursing care. Rehabilitation is directed towards assisting each resident to achieve and maintain an optimal level of self-care and independence.

 

Social Services

Traditions Health Care employs qualified social workers. These individuals are trained to identify and meet the social and emotional needs of all residents. Social workers are also a liaison to the resident’s family. Our standard of open communication gives reassurance to family members of the genuine concern and quality of care we provide.

 

Transportation

As a customer service, Traditions Health Care provides transportation needs to the resident. We are in business to serve our residents.  

 Quality Assurance

Traditions Health Care has a thorough Quality Assurance Program that allows each team member of the facility to be survey ready at all times. The program unifies team members and focuses them on a high standard of care. We meet on a monthly basis to address quality assurance issues by each department.

Culture Change

We have defined culture change as transforming the care and the buildings from an institution to a home. We insist the residents must be the voice of this culture change movement. The resident councils vote on any change we suggest.  We listen to the resident’s ideas. Culture change is mostly a thought process change.  Institutional thinking takes away from the freedoms and rights of an individual.  Culture change thinking restores freedom and individuality.  Giving freedom means letting the resident make choices about his/her care and life.  Since Traditions Health Care’s existence, we have implemented the culture change attitudes of respect, dignity, value and worth.  We actively instill these philosophies into our employees.

 

Physical Plant, Facility and Grounds Maintenance

Traditions Health Care ensures the nursing homes are a functional, aesthetically pleasing, sanitary, and comfortable environment for all residents, personnel and visitors. We pay attention to details. The outward appearance reflects the inward attitude toward care. If the outside of the nursing home has a professional appearance, so will the inside. Customers see that we take pride in the physical structure and appearance of the facility and this carries over to the care of our residents.

 

Dietetics

A quality dining experience is an essential part of a residents stay. We meet a weekly to review resident’s nutritional levels. This gives us a chance to immediately correct any variations in the resident’s eating habits. Monthly, we have a registered dietician audit each resident’s chart. The dietician conducts analysis on the resident’s nutritional needs and ensures that we are meeting our obligation to the resident. Monthly, we meet as a Quality Assurance committee to review dietary needs to ensure a higher quality dining experience.

 

Continuing Education

The training of staff members is essential in providing Quality of Care. Employees must be given the proper tools to carry out their responsibilities and to achieve our high standards. We hold training meetings at least twice a month. These meetings are designed to communicate current care giving concerns and provide procedures training. We have an in-service director who trains our staff on techniques and standards. Our management staff members attend training meetings and sessions; keeping them informed on the newest care issues and educated on new processes. The managers teach the new information to staff members.

Resident Activities

An active resident is a happy resident. Resident’s must have a consistent program that is appropriate to their needs and interests. The activity program encourages socialization and a continuation of normal activities. On admission to our facility, we identify the specific interest levels and cognitive abilities of each resident and tailor the activities to those needs. We review each residents activity needs upon a change in medical condition. We review the entire activity program monthly as part of our ongoing Quality Assurance program to address any changing needs.

 

Medical Records Administration

A critical component of Quality Care is the medical records on all residents. A medical record is a documented history of each resident’s medical needs and a record of how the facility meets theses needs. We ensure that each resident’s plan of care directly translates to the services performed. We have an ongoing auditing system that reviews each resident’s chart daily, weekly, and monthly to ensure complete and correct documentation.

 

Pharmaceutical Services

Traditions Health Care has established written policies and procedures for: pharmaceutical services, direct administration of drugs to residents and controlled access to all drugs and substances used for treatment. We have a contract with a pharmaceutical company to ensure that the labeling of drugs are based on currently accepted professional principles. The pharmacy company reviews each resident’s chart for drug utilization. They monitor the outcome of drug therapy or treatment outcomes. All recommendations are incorporated into our Quality Assurance plan.

 

Executive Communication

Success is directly connected to good, clear communication. Each morning we hold leadership communication meetings. Yesterday and today’s events are communicated to the department heads. The department heads communicate to their employees.  All employees are encouraged to share their ideas, questions, and concerns. We have adopted the saying from the Book of Proverbs that say, “Where there is no vision, the people perish”. We communicate regularly to provide leadership and vision for the nursing homes. We have an open door policy.

 

Executive Financial Management

Fiscally, we have developed a monitoring system that allows us to calculate our daily per patient day (PPD) costs. Our system evaluates each budget line item as a PPD cost. It also compares those PPD costs to prior years. We monitor how each department is managing their responsibilities. The results are communicated to the departments so they have a clear direction of where they need to be.
